Registration

Registrations and Re-Registrations may be made at the Rectory Office, Monday through Friday, from 9:30am to 4pm and 5pm to 7pm.  

Our Coordinator of Evangelization, Carmina De La Rosa, will be available Tuesdays and Thursdays, from 12pm to 8pm.

The required documents at the time of registration are as follows:

• • Child's Birth Certificate, if the child needs the Sacrament of Baptism

  • Child's Baptismal Certificate, if the child needs the Sacrament of Communion

  • Child's Baptismal and Communion Certificates, if the child need the Sacrament of Confirmation

  • Transfer Paper, if the child is coming from another parish

First-Time Registration is $150

Re-Registration is $65

Please SAVE & PRINT your Registration Form, and then return your COMPLETED Registration Form to the Office of Evangelization at the Rectory.
